Enhanced Care Management

Enhanced Care Management (ECM) is a Medi-Cal benefit that connects eligible members with a dedicated care team to help coordinate health care and other services based on their individual needs.
At The Uncuffed Project, ECM makes those connections meaningful—helping individuals navigate systems, address barriers, and work toward greater stability.
HOW WE DO ECM
We Don't Just Make Referrals. We Walk Alongside You.
Our ECM model focuses on understanding the whole person, not just one need.
We work with each member to identify their priorities, create a personalized plan, connect them with appropriate services, and continue supporting them as they work toward their goals.

1.Connect

Referrals may come through Medi-Cal health plans, health care providers, community partners, justice-system partners, or self-referral.

Our team reviews the referral and connects with the individual to begin the conversation.

2.Understand
-
We take time to understand what's happening—not just what's on a referral form.

That can include health, housing, behavioral health, transportation, food, employment, benefits, family needs, and other barriers to stability.

3.Plan
-
Together, the member and Care Manager create an individualized Care Plan based on the member's goals and priorities.

The plan connects those goals to specific services, referrals, and next steps.

4.Coordinate
-
We help connect members to providers, services, and community resources—and follow through.

If a referral doesn't work, a need changes, or a new barrier comes up, we help adjust the plan and identify the next step.

5.Continue
-
ECM isn't just a one-time referral.

Our Care Managers stay in touch, track progress, help address barriers, and update the Care Plan as needs change.

WHO IS ECM FOR?
ECM Is For Eligible Medi-Cal Members With Complex Needs
ECM is available to eligible members enrolled in a Medi-Cal managed care plan who meet the requirements for an ECM Population of Focus.
This includes individuals transitioning from incarceration, among other populations identified by the California Department of Health Care Services.
