Community Supports are services offered through Medi-Cal managed care plans that address health-related social needs and help eligible members overcome barriers to health and stability.
At The Uncuffed Project, Community Supports let us go beyond traditional care coordination by connecting eligible individuals with practical resources and services that support their everyday lives.
WHAT ARE COMMUNITY SUPPORTS?
Support Beyond the Doctor's Office
Health depends on much more than what happens in a doctor's office.
Community Supports can help address practical needs that may impact a person's health and ability to remain stable—such as housing, food, transportation, and other essential needs.
The specific Community Supports available depend on an individual's Medi-Cal managed care plan, eligibility, and needs.
HOW WE HELP
Turning Needs Into Connections
When someone tells us they need help, we don't look at the problem in isolation.
We look at the whole picture, identify the right support, and connect the individual with available services.

1. Identify the need

We start by understanding what's happening and what support is actually needed.

That could mean housing instability, food insecurity, transportation barriers, or another health-related social need.

2. Determine available support

Our team works to determine what Community Support or other resources may be appropriate based on the individual's needs, eligibility, and health plan.

We help connect the individual to the appropriate service or provider and help navigate the process.

4.follow through

Our work doesn't necessarily stop at the referral.

We follow up, help address barriers when possible, and coordinate with the individual's broader care team when appropriate.

Depending on eligibility and health plan availability, Community Supports may include services related to:​

Assistance navigating housing resources and identifying appropriate options.

Connections to food and nutrition-related supports.

Transportation

Help addressing transportation barriers that may interfere with health, appointments, or stability.

Recuperative Care

Support for eligible individuals who need a safe place to recover following hospitalization or other qualifying medical circumstances.

Additional Community Supports may be available depending on the individual's Medi-Cal managed care plan and circumstances.

WHO CAN RECEIVE COMMUNITY SUPPORTS?
Support Is Available to Eligible Medi-Cal Members
Community Supports are available to eligible Medi-Cal members whose managed care plans offer the applicable service and who meet the requirements for that support.
